Fighting Chance Support Coordination will work with you to:
Understand your NDIS plan goals and funding package
Set up service agreements with your chosen NDIS service providers as well as review and monitor the quality of your chosen supports
Build your capacity and confidence to navigate the NDIS independently
Support you to resolve and challenges with your current NDIS providers
Develop your informal or community connections to enhance your support network
Prepare for your scheduled plan review meetings, including sourcing, reviewing, and collating supporting documentation and reports
Help you submit a change of situation, plan variation or plan reassessment if required
Our support coordination team is currently at capacity and not taking enquiries. We will reopen our waitlist for clients with support coordination funding in their NDIS plan later in the year.
